About the Role:
The Outreach Worker is responsible for coordinating and supervising Peer Navigators to ensure effective HIV community outreach, health education, and service delivery. The role includes field supervision, awareness generation, HIV Testing Services, referral follow-up, data collection and reporting, stock management, HIV prevention and care support, and maintaining strong linkages with community stakeholders and health facilities to ensure quality and client-centered services.
Education Qualifications
Minimum of a Diploma in Public Health, Social Work, Community Development, or a related field. A bachelor’s degree in the same fields will be an added advantage.
Experience needed
At least 2 years of experience in community outreach, HIV/AIDS programming, or implementation of public health projects.
Knowledge and Skills
- Knowledge of HIV testing, care & treatment, prevention services.
- Strong community outreach, peer education, and stakeholder engagement skills.
- Excellent communication, counselling, and interpersonal skills.
- Experience in data collection, reporting, and record management, including DHIS2 or similar systems.
- Experience working with Peer Navigators, and priority popultaions in community-based settings.
- Ability to supervise, mentor, and build the capacity of Peer Navigators.
- Strong organizational, planning, and problem-solving skills.
Key Duties
- Lead the group of peer educators, monitoring and supervision of peer volunteers.
- Trouble shoot field level outreach issues brought out by peer educators
- Development of teamwork plan/micro-plans
- Organising advocacy meetings
- Conducting group sessions
- Facilitating the process of capacity building of peer volunteers
- Meeting with the peer volunteers weekly and monthly
- Responsible for weekly report writing, record keeping, MIS
- Recording feedback from the community GBV cases follow up, mentorship and monitoring of GBV crisis response teams
- Conducting index contact tracing and follow up of PrEP clients
- Follow up KPLHIV in terms of cohort, ART numbers, VL monitoring and update in DHIS2
- Validate the activeness of hot spots in designated location of influence using necessary tools from the M&E Department
- Distribute HIV self-test kits to beneficiaries (FSWs) and enter the data in DHIS2
- Submit timely reports to DIC Manager and District Coordinator.
